Decisions sit primarily with founders; therefore, no team member has a true definition of when their work is done, or what done even looks like! Each and every process within the business lives within the heads of those who created them; in other words, nothing is formalised and set in stone. There is no true standard operating procedure. It gets to a point where making exceptions for mistakes and wrongdoings becomes the norm, where there are no repercussions or right paths to take when fulfilling orders, or just running the business every day. 

Over time, this can lead to some serious pitfalls in the business. You'll start to notice that cash flow seems different, and margins have been shrinking without you noticing. Hiring new employees seems risky when it should be relieving. Your team looks at you every day, as if they don't want to be there anymore. These side effects may seem drastic, but I have seen them with my own eyes, going in and out of studios that are consistently struggling. Worst of all, their clients are losing trust in them, they sense the inconsistency, and don't want to risk doing business with them anymore.
